Referrers - Application for admission
General practitioners, transfer nurses, specialists and district nurses can contact us with a request for admission.
Indication and admission criteria
At Jewish Hospice Immanuel, we carefully assess each application to determine whether we can provide the right care. For this assessment, we need the medical information of the person for whom you are making the application. This includes, for example, the medical diagnosis, a brief treatment history and life expectancy. Based on the information provided, our multidisciplinary team makes a decision. In the event of a positive decision, we can often realize an admission within 24 hours. The hospice has room for 7 residents.
Prior to admission, we will have a personal conversation with the future resident and/or a family member or acquaintance. This conversation can take place by telephone, in the hospital, at home or in the hospice.
Attending physician
After admission, medical care is transferred to our team of hospice physicians, who specialize in palliative terminal care, with a focus on pain management. There is a physician present in the hospice every day, and our physicians are available by telephone 24 hours a day. In addition, they work closely with the nurses, who are also present 24 hours a day and provide customized care to our residents.
However, if the future resident needs daily care to be provided by their own GP, this is of course also possible. Additional conditions do apply. If this is a wish, please contact us prior to admission so that we can coordinate the care. The hospice doctors will of course remain available at all times for collegial consultation and advice.
